Outline:Objective: The leap motion controller (LMC) is a motion sensor that has recently become available and has advantages in terms of cost, size, and accuracy. However, it has not been fully investigated whether the LMC has sufficient accuracy for clinical assessments, particularly finger flexion and extension, which are important for diagnosis and prognosis in stroke or cerebral palsy patients. Herein, we have tested the validity and reliability of the LMC for measuring the grasp and release of finger movements. Materials and methods: A healthy participant repeated finger flexion and extension movements, and his fingertip position was measured using an LMC from the palmar, ulnar, and dorsal sides of the hand. The fingertip position was also measured using an electromagnetic motion tracker and a video. The finger movement detected by the LMC was compared with those detected by the other instruments. Conclusions: The LMC had a higher reliability in detecting the fingertip position by sensing it from the palmar or dorsal side of the hand, compared with the ulnar side. In the palmar/dorsal side measurements, an underestimation in the amplitude of the finger movement was shown in the middle range of its excursion compared to video-detected movement. However, the underestimation resulted in a highly correlated, proportional bias, especially in the palmar-side measurement, and could be calibrated by a nonlinear regression analysis. The palmar side measurement was thus the most suitable for evaluating the finger flexion and extension. We concluded that by taking into consideration the range of excursion and appropriate calibration, the validity and reliability of the LMC is sufficient for the quantitative evaluation of finger flexion and extension in laboratory experiments and also in clinical settings.

Outline:

[Purpose] Electromyography biofeedback therapy is applied to various diseases during physical therapy for motor learning. Our aim was to develop a low-cost electromyography biofeedback device kit that students could build by themselves in class and to evaluate whether this kit was an adequate educational tool for physical therapy students. [Subjects and Methods] This study included 34 volunteers from a 3 year physical therapy vocational school and a 4 year university course. All students were tasked with: 1) completing a pre-questionnaire; 2) attending basic lectures on electromyography biofeedback therapy and how to build the device; 3) building the device; and 4) completing a post-questionnaire. [Results] Mean time required to build the device was 50.8 ± 34.1 minutes for the vocational course students and 27.4 ± 8.8 minutes for university students. For both groups, there was a significant increase in the number of students stating "have an interest in," "be involved with," and "have knowledge of electromyography biofeedback" when pre- and post-questionnaires were compared. In the post questionnaire, the frequency of the "knowledge increased" response was higher. [Conclusion] Physical therapy students can easily build our new device in a classroom, and this kit is a useful educational tool to motivate learning in electromyography biofeedback.

Outline:Objective: For the convenient use of electromyography devices in physical rehabilitation during locomotion, we developed a simple, low-cost 2-channel electromyography telemeter (LC-EMGT) that can be connected wirelessly with a personal computer (PC) microphone port. The aim of this study was to verify whether the performance of our LC-EMGT fulfills the need for monitoring EMG during locomotion of patients in rehabilitation. For this purpose, we compared the performance of our LC-EMGT with an existing EMG device (NeuropackΣ). Methods: Muscle activity of the left and right vastus medialis were recorded when the participant repeated a standing-up movement for 10 seconds at 55-bpm speed. The EMG signal was simultaneously recorded by the LCEMGT and NeuropackΣ. We compared the waveform of these EMG signals and their root mean squared signals in appearance and by cross-correlation analysis. Also, we monitored the orthopedic patients’ EMG waveforms during standing-up and ascending stairs. Results: The cross-correlation analysis demonstrated an approximately 170-ms delay in EMG measurement by LC-EMGT due to the wireless signal transformation. Meanwhile, the amplitudes of the waveforms in the LC-EMGT were almost equal to those of the NeuropackΣ. In addition, with the LC-EMGT, we could visually monitor EMG waveforms of the patients during standing-up and stair climbing on-line using a PC. Conclusion: Consequently, the LC-EMGT was as reliable as the existing EMG device at monitoring EMG signals and is available as an EMG monitor in real-time. The LC-EMGT is cost-effective, convenient, and may be generally used in various clinical and sports situations.

Outline:We investigated the characteristics and recovery process of pelvic obliquity during gait and the motor functions of patients with or without Trendelenburg inverse gait, before and after total hip arthroplasty (THA). This study included 18 women who underwent unilateral total hip arthroplasty; informed consent was obtained from these patients. Before the operation, patients were divided into 2 groups: 8 Non-Trendelenburg inverse gait patients (NTI group) and 10 Trendelenburg inverse gait patients (TIgroup). We performed 2 analyses at before THA and at 2 weeks, 4 weeks and 6 months after THA. The first involved sthenometry and measurements of the range of hip abduction-adduction motion and trunk lateral bending; the second involved measurement of pelvic obliquity and the range of hip abduction-adduction motion during the stance phase in gait. Preoperatively, the range of hip adduction motion was significantly lower in the TI group than in the NTI group. In the TI group, abduction muscle strength of the affected hip side decreased at 2 and 4 weeks. At 6 months, the affected hip side showed joint adduction muscle weakness in the TI group as compared to the NTI group. Before operation in the TI group, restrictions in the range of hip adduction motion of the affected hip side may cause Trendelenburg inverse gait. Furthermore, compared with the NTI group, the TI group showed delayed recovery of hip joint abduction in the affected side as well as decrease in the hip joint adduction muscular strength of the affected side.

誘発筋電図による促通・抑制試験における最大振幅指標とRMS値指標の比較

Research Results Outline:ヒトの中枢神経系の特性を調べるために、MEP(運動誘発電位)やH波などの誘発筋電図がしばしば用いられている.これらの手法は、誘発筋電図を誘発するためのヒトの中枢神経系の特性を調べるために、MEP(運動誘発電位)やH波などの誘発筋電図がしばしば用いられている.これらの手法は、誘発筋電図を誘発するための試験刺激に先行して与えられる、ある一定の潜時をもった条件刺激や運動イメージにより変化する誘発筋電図...ヒトの中枢神経系の特性を調べるために、MEP(運動誘発電位)やH波などの誘発筋電図がしばしば用いられている.これらの手法は、誘発筋電図を誘発するための試験刺激に先行して与えられる、ある一定の潜時をもった条件刺激や運動イメージにより変化する誘発筋電図を捉え、それを指標として特定部位あるいはシナプスの促通や抑制を調べる.誘発筋電図の変化を捉えるための指標としては、最大振幅値を用いる場合と、一定期間の積分値を用いる場合があるが、この両者の差異については検討されていない.両者で差異が存在した場合、これまで同一として扱われてきた試験結果について、用いた指標に応じた解釈が必要となるため、確認しておく必要がある.そこで、誘発筋電図による促通・抑制試験における最大振幅指標と一定期間の積分値(RMS値)指標を用いた場合の結果を比較することを目的とした.運動誘発電位の多くは、振幅方向に相似であることから、両者に差異は存在しないものと考えられる.記録データは、健常者5名の右側短母指外転筋(APB)のMEP波形を用いた.各被験者において運動イメージの異なる40試行分のMEP波形を取得した.被験者毎に磁気刺激後20msから50msの波形を切り出し、同一試行中の最大振幅指標(最大値‐最小値)と切り出し期間のRMS値指標を算出し、相関係数を求めた.相関係数は、各被験者において、(0.01、0.01、0.01、0.4、0.5)となり、相関は認められなかった.本研究より、今まで同一として扱われてきた最大振幅指標とRMS値は、誘発筋電図のうち少なくともAPBのMEPにおいては、大きな差異があり、別の解釈が必要であることが分かった.今後、積分期間による差異や別部位のMEP波形の差異、H反射などの別の誘発筋電図などにおいても調査し、差異の原因やそれぞれの指標における解釈の方法を検討する必要があるものと考えられた.
